Empowering Your Journey to Success

TBI Pathways Transformed, LLC
Hope. Recovery. Skills.

Our Mission:

"At TBI Pathways Transformed, our mission is to illuminate the journey from injury to independence. By leading with hope, anchoring our care in in-home support, and building functional skills, we empower survivors to reclaim their lives and transform their futures."

We believe that the home is the most effective setting for meaningful recovery. By meeting survivors where they live, we reduce the stress of clinical environments and focus on the practical, real-world skills necessary for a successful transformation.
What We Offer
By focusing on in-home support, we are offering a service that is both highly practical and deeply personal.

1. Life Skills Re-integration
-
The Focus: Practical daily living in a familiar environment.
-
The Description: We don't just teach skills; we help survivors apply them where they matter most—at home. Our specialists work alongside clients to master Activities of Daily Living (ADLs), such as meal preparation, personal grooming, and household management, ensuring they can navigate their own space with confidence and safety.
2. Cognitive Home-Mapping
-
The Focus: Personalized memory and organization systems.
-
The Description: Every home is unique, and so is every recovery. We design and implement custom cognitive support systems—including visual schedules, medication management routines, and organized living spaces—that turn a survivor's home into a tool for success rather than a source of confusion.
3. Community Mobility & Safety
-
The Focus: Bridging the gap between the front door and the neighborhood.
-
The Description: Recovery extends beyond the walls of the house. We provide guided support for community reintegration, helping clients safely practice essential skills like grocery shopping, using public transit, or visiting local parks. Our goal is to expand the survivor's world, one safe step at a time.
4. Family Empowerment & Respite
-
The Focus: Supporting the "hidden" survivors—the caregivers.
-
The Description: A TBI affects the whole family. We offer natural supports education and training for caregivers, providing the tools and professional respite needed to prevent burnout. By supporting the family unit, we create a sustainable foundation of hope for the long-term recovery journey.
5. Neuro-Functional Home Assessments
-
The Focus: Clinical expertise meets home safety.
-
The Description: Our team performs detailed environmental evaluations to identify potential hazards and recommend simple modifications—such as lighting adjustments or adaptive equipment—that promote physical independence and fall prevention. We ensure the home environment actively supports the survivor’s evolving physical and cognitive needs.
